Description:	This project is for a smart form that will send 3 e-mails when the form is submitted. The form will have basic fields such as name, address, contact e-mail address, how many employees. This form needs to be html based so it will be easy to modify if new fields need to be added or if fields with drop down options need to change. Once the visitor hits submit. An e-mail well be sent to the e-mail address entered in the contact e-mail field with a preset e-mail message and subject line.
