Description:	we are a print comapany. we do custom apparel and cardstock prints for bands, clothing labels etc. right now on our site we have a simple "quote" form made in php. all it does is collect user inputted info such as name, email, type of job, etc. since we are redoing our entire site, we needed a complete new form. this time an order form. it needs to have 3 pages. first page consists of shipping addresss, billing address, and project info. second page consits of the details of the order, how many colors on the print. what kind of shirt. what kind of ink etc. the 3rd page is just a page that says thank you for your order # _____. and will provide them with info on how to upload us the artwork. I don't need bells and whistles. we have our website made up as is. and right now i just need the form implemented in it. There will be no credit card authorizing, or payment plans. what we do is through paypal. so at the end on page 3, it will tell the client to await payment link. the only other thing is the form must be emailed to us, and to the user's inputted email address, as a confirmation to the order. we have a few local designers that want a stab at it, but i remember using this site when i first started freelancing and i definately wanted to give back to the people who helped me. if you have any questions just contact us through here.
