Description:	We are a non profit organization that provides free surf camps for children with autism. Need some changes to a php/mysql form and database web panel control panel See form here http://www.surfershealing.org/signuponline.html Form changes 1 Name needs to be broken into first and last name Under surfers info a ) Childs last name if different from parent field to be added Delete release form signed field Add 1 paragraph of text Web content management. Database needs to automatically assign the time for a surfer. This will change depending upon camp parameters set such as hour of camp and number of available slots. The times will need to be assigned in 15-minute blocks such as 12:00, 12:15, 12:30, 12:45, 1:00. Under add application a drop down box needs to be added to select which camp to add application too. Under print application function needs to add 3 fields and delete 2 fields for the camp schedule. Confirmation page needs to have a link to the liability form added. Email address the form is sent to needs to be changed.
