Description

I need the forms on my editing website to work

The first form is a refer-a-friend form - form already made just needs to be coded
The second form is a comment/testimonial form- form already made just needs to be coded

The third form is an order form and a little trickier.
I need this form to multiply the option they chose by the number of pages they entered, and for this number to show up on the next page, then i need them to confirm that amount and it to lead them to a receipt. I need you to make the receipt page and the confirmation page, but the form is already made. The code you make must also support checkboxes and uploads. There must also be a restriction so that if certain areas werent filled out it wont go through and notifies them of it.
