In the last several years I have branched out into website development (ie, software development). For examples see http://www.goldco-online.com/ , a fully custom dynamic database driven e-commerce with shopping cart site I authored and also a (demo) content access subscription validation and billing site with recurring billing capability at http://www.electronics-software.com/BillSite/BillSiteTest.php . Both of these example sites have multiple form pages. The actual work would only be very few hours, but I am allowing for time to communicate, etc. This bid is for a single php script with template for the form including setting it up on your server and verifying functionality.
