Description

I require a php coder who can help me out in php programming. I want to create 4 forms in php which would connect to mySQL databse. These forms can added, edited, saved in excel and should be able to get printed by valid user whereas the admin will have all above user privilages plus he will have the facility to add or delete new user and new form.

I have came across a tool called as Form Tool which I like the most for managing the forms but it doesn't allow you to edit the form which you created, so if you can put it all features of form tool with some additional features which I require in it.

Note:- The form will consists of select option which should be updated from the backend i.e. from the database. There will some textbox calculations which I would provide for it, you just have to do the needfull validations to it using those calculations.

My budget is around $50-$75 for this project. I will be providing you with the html form for it, you just have to code the form in php and also maintain it through database and in the front end also.
