Description
I am looking for a developer to make a multi-page (7) form that will have the data collected sent to an e-mail address. The total <form> items to send to e-mail are 5. That means I collect something like: name, e-mail, url, phone and age.It must use session_start(); to carry over the information from one page to another. And it must also check if visitor tries to enter the form at a different page then page 1 and then redirect the user to page one.
The values collected are simply sent to an e-mail address.
The form must contain: captcha and a time delay for the last SEND button. More in the attachment.
I will pay for this 50USD. Escrow payment is no problem. All work will be done on my server. I will send you a server location for test including FTP access.
I will provide all HTML pages and PHP code needs to be added.
