Description:	This is a custom scheduling application for a pediatrician's office that offers various forms of parent coaching (group, individual, for teachers,etc). It requires some modifications: *Need to send email reminders a day ahead of time for people who have signed up. So this would require a script to actually send a form email, and configuring a cron job on the server (via a Plesk web interface) to run it. *The addition of a checkbox on the registration form for the public user to show that they've read the parenting agreement (this would just be a link opening up in a new window). An additional field would need to be added to the database table to show that this was checked/not checked. *The addition/change of the form fields for the public user to include thefollowing: Parent 1 name employer/occupation Parent 2 name employer/occupation Children's names and ages (just a big textfield) home, work, and cell phone #s (3 separate fields) patient of the practice?
