Description: Hello,

I have an online employment application in PHP. It works very cleanly, but I've decided that I'm unhappy with the output.

When the form is submitted, the script uses a Word template to send an email that formats the responses to the question. The email goes to both the applicant and to myself.

The problem is that the formatting is pretty good, but I would prefer to receive the same type of printable "form", but by PDF instead.

So, I'm looking for someone who can take the current form output and make it PDF attached to an email instead of the current result.

Also looking to modify the spacing on 1 question and reword one question on the output (the form input is all 100% great).
