Description

I need a form, that when filled it, runs queries and comes back with an answer on each section. To explain more, read below.

our clients give us credit card information. ie, name of lender, start date, balances, limits etc
depending on the information, the contract may be unenforceable. If it is, we challenge the credit card company and get the client a refund.

we need a system that gives a rough review of the information
so if for example, someone fills in:

Type of credit: Loan
Lender: Barclays
Start Date: 05/02
Balance: £5643
Limit: £7000

They also complete their name and contact info, which is all emailed to us

upon filling in the information, they click submit, they get an auto answer of "yes we can challenge it" or "no we cant". we also get an email with their contact information and the results of the form.

Credit cards dont need the limits completed, however loans do. So what we would like is first an option of "how many agreements do you have". Then it shows the form, each section starts with "type of credit" and when chosen, it shows the required information we need.

This would be an interesting little project for someone and we will be happy to offer more work in the future if we get the right thing for the company.
