Description

Should be a quick fix (less than 1 hour max) ---

Program was made for me 2 months ago and cannot find programmer....
It is in HTML and perhaps PHP and stored on my cpanel.

Program flow:

1. Visitor reads form and decides to signup so they can be notified when our new website comes on line.....

2. They fill out 3 fields of informaiton:
a. Name
b. Email address
c. Provide comments or question about our new site

3. When they push submit the followng happens:
a. Visitors is sent an automatic email telling them we got their input and we will notify them when the site is reay for their review.
b. the data input is stored in database on MySql
c. An email is sent to , providing all the input data from the visitor.

Not sure why it is not working....perhaps it has something to do with MY SQL? Cpanel did go through an upgrade by the host so I need someone to be sure that the program doesn't have to be fixed everytime an upgrade is made to cpanel ----

Need fixed in 24 hours...

This will buy you lunch but the big value is I have alot of projects to do so I am looking for someone who works fast, speaks very good english, reads english very well, and doesnt have a history of misunderstanding their customer which is often due to language barriers.
