I am in need of the following upload form script to add to my website:

-Users need to enter their: *Order Id #, *Name, Company Name, *Email, and Telephone

*required fields

-Then there needs to be 7 upload fields where you can browse and add a file to each field. So 7 total files can be uploaded to our ftp.

-The uploaded files CAN ONLY BE: gif, jpg, zip, psd, sit, tif, tiff, eps, ai, fh, cdr,qxd, or pdf. Anything else and it displays a not accepted file warning.

-When they hit submit, there is a status bar that shows the progress of the upload (perhaps speed, time remaining, etc).

-The total upload (up to 5 files) is created in a new folder on the ftp with the Order ID # as the folder name.

We need this to be created fairly quickly, so please bid on amount and time to complete.
