What I need Completed

1. Need the 1st option to have addition field to select zip code for origin.
2. If user selects State it already goes to a pull down with cities in that state. Now if the user inputs a zip a different step will appear that will ask for the radius

Also need seperate radius form not in dynamic page.
Use as clone: http://www.rightnowloads.com/zipcodescript/Example/example2.php

Modify last form on page to have:
1. Select State first or zip
2. dynamic pull down of cities based on state
3. Input radius
4. results page should show loads with origin cities that match origin cities in that given radius.
