Description:	This project is FORM FILL PAGES: -SINGLE PAGES- This will be a PER PAGE/SITE price. (please see bidding rules before making a bid) 1. Seeking a HTML/XML form fill page. These pages will not be a blank form fill page but a designed page. Therefore it is desiging a single page with graphics/header/text. 2. The form fill must be functional and price must include the putting of this on the server. ( you will be provided temp. access to said). 3. All form fills need to be done in php or cgi. (please PM which you perfer and why). 4. These FORM HTML Pages will be used as stand alone sites for Google payper click, and also for within websites. Some websites may have multible form fills. 5. Prices should be very fair and based on a. installation service of forms. b. Design, creation work of form. c. technical creation of form. d. forms, html, and code all will become the property of this buyer, or website owner after final payment is received. 6. First 3 must be completed with IN (10 days). 7. 99 percent of sites will be linux, I will expect that .asp work would be a bit more and will be discussed prior to work begining. 8. Some designs will be more complex then others please base your price on medium to moderate complexity and we shall hope that overal will work out. 9. I am not LOOKING for a self generating code page or a blank page with form fills. These will be NICE LOOKING, APPEALING forms with in a HTML/XML compliant page. That will be submitted to an e-mail address. important notes: A) I have been attempting to find reasonable associates to do gradually increasing amounts of web design. I have more then 10 personal sites I want to have done, and am obtaining clients on a routine basis to aquire more work. B) I am definetly not opposed to working with overseas associates and frankly would prefer this due to an increased work ethic. As well as price consideration. C) I would become the american face to an overseas team. D) I have one associate who does great work and want to have access to several. I hope to create with in 12 months a large amount of volume of steady work. E) The first 24 months PRICE will be an issue.. and this is why I am using GAF. Bidding RULES: 1. All clients are our property 2. All code/design are the property of the website own once paid. 3. we pay 1/3 deposit , 1/3 at complete, and 1/3 after all documents, code, files, and installation are complete. 4. If contract is termated by web design/coder all deposit, and any other payments shall be returned. In addition if designer/coder is unable or unwilling ot complete this project they aggree to pay us to relist this on GAF to include the $25 dollar fee. 5. YOU MUST SAY YOU AGGREE TO ALL TERMS BEFORE YOUR BID IS ACCEPTED. 6. All designers must have the ability to communicate reasonably well in English. We perfer to communicate via SKYPE, AIM, YAHOO, and e-mail. I use PDFs and provide rough drawings to give you an idea of what I am looking for. 7. All clients/customers are the property of USABIZ01.. you are not allowed to attempt to go direct to our clients. note: I am a loyal person, and I do not judge my teams in any way. I am looking for motivated, creative, honest, designers who want more work. I expect you to keep your word and keep to deadline promises. We will be reasonable in understanding some delays however consistant delays is not tolerated. Again your bid is for a quality graphically designed, form page to be used to capture customers/prospects information.
