Description

Looking for a freelancer to do a simple web site development for Faculty Evaluation which include students, head, human resources, faculty ( for self-evaluation ) and admin.

there are pre-defined sets of questions for each category such as personal characteristics, communication skills, mastery of the subject matter, instructional methodology, class room management, etc.

answer selection is 1 to 5 ( option button )

need to create a ranking per department, faculty and evaluation results report per faculty member.

I don't have much time for this and in tight budget.

This project is very simple since it is like a survey and the code needed is easy for those experienced PHP/MySQL.
