Description

THIS WILL BE A HOSTED SITE: WE NEED A SITE WHERE USERS CAN SECURELY LOGIN AND GO TO A FILL-ABLE FORM SECTION. AT THAT POINT THEY NEED THE OPTION TO FILL IN SEVERAL FORMS (WHICH WE HAVE) WHICH WILL POPULATE THE MYSQL DATABASE,(WE WILL WORK TOGETHER AT THIS POINT) THEN AFTER THEY SUBMIT, THEY NEED THE ABILITY TO PRINT THE FORM AND/OR EMAIL IT. ALSO, THEY WILL NEED TO BE ABLE TO LOG BACK ON AND SEE/PRINT A HISTORICAL VIEW OF FORMS THEY HAVE SUBMITTED.

ONLY SERIOUS & SKILLED BIDDERS PLEASE. MUST HAVE VERY GOOD ENGLISH COMMUNICATION SKILLS.
