Description

I need to have an email form from a template monster template updated. I need to have the form updated in 2 locations so that it will email the inputted fields to a specific email address. This is a very simple fix; and will probably take a skilled person only a few minutes. The form should NOT require the senders email. This should be a server side transaction. If the file type needs to be changed to PHP; let me know. Let me know what needs to be done to make this a simple online form.
