Description

I posted this before but I did not have the files ready and no one was getting back to me, so in the interest of getting this done ASAP, I have reposted with the attachments for you to peruse.

There's 2 forms there, and a list of menu items and email addresses. Quite simply, I need this form to

1) work - I need it to email me the data and file attachments. I would like security feature so that .exe files cannot be send and the file size is limited to 50MB.
2) In the list, there's items that are in a pop up menu in each form. For each item I would like the form to be sent to a different email address.
