Description

Please bid to create an PHP order form for us that must work with PayPal Website Payments.

You must design and create the form exactly as shown in the Image File above.

Important notes;

On Step 1 of the form, we will provide the Kanji Symbol Image files which measure around 150 x 180 pixels each. You will place these in the form accordingly. Style 1, Style 2, etc

On Step 3, Customer will select order total- eg;
1 Word / Phrase $10, 2 Word / Phrase $16, 3 Word / Phrase $22, etc
The order total customer selects here must be reflected on PayPal.Com payment page correctly.

Mandatory Required Fields
Step 1 > Select a Print Style
Step 2 > Select a Font Style
Step 3 > Word / Phrase 1
Step 3 > Select Order Total
---- ( Word / Phrase 2,3,4,5,6,7,8 - Optional ) ----
Step 4 > Name, Email Address, Re-Enter Email Address
---- ( Hear about us, Newsletter sign up, Comments - Optional )---

If any mandatory fields not filled correctly, customer should be prompted to enter details before checking out to Paypal.

When customer click checkout button, 2 emails should be automatically generated and sent out.
One copy to Website Administrator Email Address
One copy to Customer Email Address

The Email must display all data that was entered on form 1 Per Each Line neatly and must have our logo at the top of email. We need to also enter some additional text in the email to customer apart from standard form data.

Please have past experience with creating such forms and PayPal Payment / Checkout.

We will provide you our PayPal Buy Now Button Code, you must handle the rest of tasks.
