I want a website similar to this one.

I need a site that allows users to search for a clinic by name, location. Including zip code search by radius.

Registered users are allowed to submit their clinic with their information including a photo.

I would like for this site to be nice.
I can buy a script already made.
But I would prefer you to find a script. Customize the looks for me.
If someone wants this project I will give them the name of the site so they can put a logo at the top.

Also I would like registered users the ability to rate the clinic.

I am also giving programmers a chance that have no rating. But I have to be extra careful with users who have no feedback.

One more thing.
Security is important.
If possible image verification for forms is a plus.
