Description

I would like some forms added to my Joomla website

1. I need one form added to the homepage with these fields like this site: http://www.dotster.com/design/studio/freequote
Name (required)
Email (required)
Confirm Email (required)
Phone (required)
Brief Project Description (required)
Touring Number (Need this to prevent spam)

2. I need one form added to a project requirement page exactly like this one:
http://www.finerdesign.com/website_design_quote.php?goto=advanced
It must have a touring number as well to prevent spam.

3. Need one form for a promotion I'm running:

Same as the home page form but need to add a drop down field: Which promotion are you interested in?

The site is based on Joomla so you must install and test it to ensure it works correctly. I don't want to use a Joomla component I want the forms embedded on the pages like HTML and it should call to a PHP form. I've done this already but need someone to make the forms above requested to look more professional.

Budget is $30 total for the two forms
