Description
I need a simple, yet secure web-form that can send attachments and uses CAPTCHA style image-security code. PHP.see example here http://motorwebsites.com.au/contact.php (though this version cannot sent attachments but does check fields and uses captcha security.
For the attachments it needs to have file restrictions (ie. "allowable extensions are: .pdf .doc" all other file types will cause an error. (for example only)
Results sent via email with CC and BCC options with attachment also preferably storing form details into excel/csv on server
This needs to be easy it set up (for non-programmer), have the ability to add additional fields if needed, and provide decent security against spamming.
I need it ASAP. Thankyou
