Description: Need a custom shipping module made for zen cart.

I just need 2 radio buttons and 1 text area

radio one: UPS
radio two: Fedex
Text: type you account number
(leave empty if no acct number)

this doens't have to process shipping methods or price. I just want to know if they have fedex or ups and what their number is.
