DO NOT BID ABOVE $125.

Want template or design of site to have:
1) searchable forums. As admin I would like to have featured forums. Want to be able to ask for ideas to improve site and other functionality.
2) groups or communities which people can make. Then others can join. Can also invite others to join.
3) Blogs
4) Simple header design.
4) Front page have link to form to complete, see attached file: 'Form need filled out for jobs.html'. This form just captures all the info in an email. Allows up to 5 email addresses where this form info goes to.
I have html knowledge, but not php knowledge.

DO NOT BID ABOVE $125.
WILL PROBABLY HAVE YOU POST TO MY SERVER.

Need this form to just output in an email all this information.
Subject: Ideas and Passions Job (subject of email)
In the code or admin need to be able to add as many email addresses as needed for this information to go to to track jobs.
Need date stamp of when job input.
