Description:	Data entry program to MY SQL We are in need of a very small application that a) Logs the user in .. (allow multipel users to acccess the Data base , at the same time ) b) offers the user a page to enter data into Data is sorted by user logging the data and time of each and every entry is needed. The data to enter by the user is ---- a) Name of Artist / Band alpha numeric 60 characters max ---- b) Contact name alpha 40 ---- c) Contact email as it sounds ---- d) Verified email Y/N - default no ---- e) Location ulrs were found alpha numeric upto 200 characters More then one ---- f) music styles Tic box - imput ( see picture ) as per the artests descriptions not the users Further details of the list can be found at www.proudmusiclibrary.com/en/index.php ---- g) Contact made Y/N - Default no ---- h) phone number 20 numeric ---- i) mobile number 20 numeric ---- j) Country of origin Drop down box ---- k) The artists bands web site 200 alpha numeric c) IMPORTANT You will not allow an entry if its is logged already , Verification is by email entered ( checks whan entering ) , BUT important , if you get a duplicate attempt , I want this logged as an attemp in a seperate file , by the use , for the user to see d) Report , you will export the data by the following reports `1- Screen report , By user showing how many they have entered , and on what dates 2- By double entries soreted by use and dates 3- Export , the completer file as a CSV , allowing Me to select the fileld I need to export e) A mail merge report allowing me to Write a mail out to the cleints entered , that includes ---- a) their contact name b)above ---- b) the artist / band a)above ---- c) Where we found you e)above ---- d) natually the email address to use NOTE a Data Entry User can only see what they have entereed , but admin can see them all .// Payment will be via gaf 50% on demo to your server , 50% when supplied to my server with Source Have fun I want to award this with in three days
