Description

I need 6 submission forms installing on my site. I have all the code it just needs someone to install on my site and change the CSS.

Please see the attachment for full details and you will see what I require.

FTP access will be given then when completed payment will be made within 15 minutes of completing the job to my satisfaction.

Previously the forms were created with formtoemail. That is OK for me.
I will pay $60 for this project.
