Description

We want to complete a task for our client. We have enough tasks, so our coder can’t complete this task our self. We are giving all details of our client below:

I need 5 forms developed using Facile Forms for an existing Joomla! web site. Here is an overview of general requirements for each form:
1. Each of these forms will require that Credit Card information be gathered. I would like the last 4 digits of the credit card number stored in a separate database base field than the rest of the credit card number.
2. These forms will need all the field information sent in a nicely formated email.
3. Only the last 4 digits of the credit card will be emailed.
4. These forms should be broken down into multiple pages to by section to prevent having to scroll so far.
5. CSS formating should follow the same as the current site.

All but one of these forms are currently html forms and can be viewed online. The name of the five forms are: Convention Exhibitor Membership; Convention Attendee Registration; Convention Exhibitor Registration; Membership Form; Monthly Meeting Registration. I have attached the details for each of these forms in the pdf's attached.

Note: I want to start work today and want to complete Monthly Meeting Registration form today. When you complete first form successfully, we give 25% payment to you and create escrow payment of 75% and i will release remain 75% payment to you after complete all task

Our budget between $100-$150
