Description

I need an application form to gather information about volunteers for my website which is Drupal 5.x based.

The form includes attachment fields and those attachments must be sent to the email the form goes to.

The form is multilingual (4 languages).

I require you build the form on a test site provided by you before installing it on my site. The form must be fully functional.

Payment by escrow 50% at the begining after installing the test form and 50% at the completion of the project installed on my server.

The site is on a shared host.

This is basically what I need:

-Form to gather info in a Drupal site
-Attachments sent to an e-mail
-Multilanguage

I need someone able to communicate with me often during the project, basically through email.
This is a quite simple project but needs a good knowledge of Drupal. Please only apply if you understand the requirements.
