Description

I have a preexisting order form that I need converted to a multi-page web form that will validate e-mail addresses. I need the form to calculate order totals and multiply be a certain percentage to determine deposit. I need to allow customer to select three payment options (2 were they simply input credit card information or bank information and the third where they can pay via PayPal. Also need form results sent via e-mail.

Form fields and layout have already been created just need working code.
