Description:	This is a rush project. Small but need it fast 1 day turn around. Form is basic 3 options each Part One: loan amount type of loan rate your credit <--continue---> Part Two: Name Phone Number Best Time to Call <---submit---> Form is loaded into the pages using an iframe. must use the attached css file. the initial "Demo" page of the website (client view) is attached - This formatting and layout must be used for proper layout. delivered layout must be installed on the server and fully functioning. As it uses an iframe, it is a simple process of the single file changeout and setup of the form to go to the appropriate thank you page upon submit.
