need this php/cgi form edited (the cgi only)

to take uploaded image and attach to email that is sent as notification. Right now the script sends an email with a link to image on server. My client want it attached as well.

The upload.cgi is attached. For an experience perl scripter this should take 15 to 30 minutes.
