Hello Everyone,

We are looking for someone experienced in either Javascript, Perl or PHP who has in the past built or transformed already designed online forms by inputting extra code to create form fields (checkbox, radio, input, etc) to calculate and display a total within a text div area.

This is pretty simple I bet but quite important. On our form there is about 10 different fields who need to only add up to a specific plan and/or package. No major calculations.

Example of what I'm looking for is similar to what the order form is at http://www.xhtmlit.com/Get-Started (that form might even have more complicated calculations because of subtraction and additions, etc..)

Time line is pretty strict for this, please make sure that the time line you define with your bid is as promised, NO LONGER. I will cancel the project and let someone else earn the dough for their honesty.

Budget: I'd like to keep it around $50 USD.
