Description

Hi,

We would like to be able to place a link on our site, on lets say the payment pages, and a few other pages where customers may have a problem and this link when clicked would send us an email instantly with the url of the page where the customer was on. This is so we can take a look if we get enough problem reports and we can see if there is an error. After the customer has clicked the report a problem link we would still like a popup window to open with a comment box that would say something like, Please Explain Site Problem Below: and then below that it would have a comment box. Once a customer click submit we would also get that email again with the page url on it.

I would also like a spam security measure on the pop page as well. I think they call it a Captia or something like that. They have to type in what it says on the screen before they can submit.

We would like this done as soon as possible so please be able to complete the project within 3-4 days from winning the bid.
