Description: PLEASE READ BEFORE BIDDING!
I need an RSVP for. You can create this form in HTML, PHP, Flash, MySQL. I already created the design and how the form will look like, so your job it is easier now. I want the to be able to add, edit, or remove fields in the form. I know some HTML, and some PHP and MySQL, so I can install and modify a few scripts, but I cannot create them. In other words, you can send me details on how to add, edit, or remove fields in the form. Details as step by step or you can make it more easy by creating that function in the admin area. Administrator login with user name and password.

This is a quick project - it will be completed within 1-2 hours. I am bidding $30 because it is the lowest bid possible on GAF - it can be done even cheaper ;) If you want to modify fields in the form, I will create additional form in admin area which will allow you to add/edit/delete fields in the main RSVP - no modifications to scripts or database will be required.
