Description

This project needs to be completed by Sunday evening MST (North America) at the LATEST.

I have a survey as a word doc (see attached). I need it to be a link in an email we send out to clients after they have attended a training session. We can host on our website and I can provide a dev environment for you to work in.

- Survey must be in Joomla
- Number options will be radio buttons
- I would like the survey dolled-up a bit, background colour, logo. etc.
- Please suggest simple design (view www.guild1.com as a style guide)
- Timely completion is essential!
