Description

I need a simple form made for a website requesting name and email address in exchange for an ebook (which I will provide). There should also be a picture of the ebook on the form, and I will provide the image. Once a visitor has completed the form they should be sent direct to a page with a link to download the ebook on it. Email addresses and names captured should be forwarded to a specific email address for keeping.
The form needs to be a popup (over) appearing after 15 seconds and also when leaving the website. It should only appear once to each visitor ie. not every time they visit another page.
I need this done very urgently and it will need to be completed by end of play tomorrow (Thursday)
