Description

I need someone to get this:

www.discountworkouts.com/new/mailchimp.htm

to work here:

www.discountworkouts.com/index.php

with mailchimp.com integration and added features

1. Once users submit the form successfully the form should not display on subsequent visits to the page.
2. The form needs to integrate with Mailchimp and use ajax when validating and submitting. Users should not leave the home page after sumbmitting the form. All errors and submission thank you text must display using Ajax with no page refresh or additional navigation.
3. using the 'jquery validat remote' function, the form should check the mailchimp database for the email address and show "email already registered" if it is found in the databse (enter someone'at'somewhere.com in the sample form above for an example).
