Description

Highly-rated, high-competent provider wanted for the following project:

Create a .php based web form that collects two of three optional fields of input data from the user. Data collected from the user will be used for a http get to retrieve structured data (some in XML) to populate a web form. I will provide structure and layout of the web form that the user can complete and print (print friendly layout required) using window.print().

On the initial http get, there will be some logic required. The http get will include a date (eg, 8/3/2009). The form can only be generated within 21-days of that http get date. If outside, it will return a brief message on a new web page.

The output form itself may have some logic, input fields so please bid accordingly. A unique code (based on timestamp or other unique variable) for each form generated is required. The unique code and one piece of input data should be captured in a flat-file or database.

In the end, the provider's output should include four pages: one page to collect the data, one error page if the data collected is incorrect, one page if the data is past 21-days and one page with the editable form.

PROJECT REQUIREMENTS:

-Provider can recommend best solution including languages use for this project.
-Code should be clean, minimal for quicker load times and commented.
-All inputs should be sanitized/parametrized.
-Should use existing cSS for any text or formatting management.
-Must be a cross browser solution including IE/Safari/Firefox.
-I will provide template, which includes header, left navigation and footer. All new pages should fit within this layout.
