Description:	1. General description - Simple PHP webpage with Javascript. 2. Environment - PHP4, JavaScript 3. Task description - We need 1 PHP Webpage with Javascript. On this webpage I want to build 1 web-form based on simple MS Excel sheet that we will provide for interested people who already have similar experience. Webform includes few product positions, price per product and quatities. Visitor needs to calculate price automaticaly based on entered quantity and after click on "calculate price" button. We need 1 more button with "send e-mail" name which sends calculation as formated html e-mail to visitors e-mail. 4. Delivery requirements: - open source, non-coded script with code documented inside containing comments about functions and variables in English language. - read-me file with short description on how to add new positions, change price for products, change redirection url - copyrights messages which allow us to change the code in the future 5. Additional information. If you have any questions or something is not clear enough then don't hesitate to contact us.
