Description

Hello I am seeking a PHP programmer to program a web registration form that submits to MySQL database, emails the form data to the website administrator and emails an autoresponder to the registrant (person who submitted the form). In addition, seeking a way for the website administrator to login to an admin panel, sort data, and export the data to an excel spreadsheet.

Can you do this?

(*) Setup Web Registration Form (17-20 Form Fields)
(*) Setup MySQL Database
(*) Setup Password Protected Admin Panel (user can easily sort, select, edit, delete and export registration data as Excel spreadsheet)
(*) Anti-Spam Protection (Captcha, etc.)
(*) Registration sends thank you autoresponder email to registrant (confirming their submission).
(*) Registration email is sent to the website administrator with Registration form data.
(*) Data gets stored in MySQL datbase.
