Description

No automatic responses. No "see my profile" response. Results only, please. The first solution that works and looks good wins. Responses without attachments will not be opened. $70 flat fee will be paid to winner.

Project -
1. A webpage that list the contents of a folder. The folder is named "uploads". The webpage is named "index.htm" or "index.php". The webpage and folder are both at the same level. The table has a title of "Project Documents" (size is H1). (Include 5 lines in the final project and provide 5 small files in the folder named "uploads" - Just small text files are ok).

2. The table has the columns "File Name", "Description", "Author" and "Date Posted"

3. Actions of the table: Any file on the table can be downloaded from a hyperline of the title or line. Javascript roll over highlight would be a nice addition.

4. A button at the top of the table is named "Upload A File". Clicking this brings up a form to upload a file to the folder "uploads". The form should include the fields "File Name", "Description", "Author" and "Date Posted" and a button to browse for the file the user wants to upload.

4. Upon clicking the link, a "browse computer" dialog comes up (via standard PHP) and allows the user to find the file. If they abort, nothing bad happens. When they select the file and all the form fields are filled in, the file uploads to "upload" folder. The script returns the user to the file list table and shows the new file listed.

Again, this is a simple PHP/HTML task (no SQL database) and I don't want to spend a lot of time negotiating bids when I could either search the web for the script or write it poorly myself.
