Description

Hi,

I need someone to urgently and very quickly (within 45mins) take 2 PHP pages hich I have created. They have a form, and when a person selects something from the first list field, the next list field needs to select and list the ones that match to the previous list from our mySQL database.

They then need to get it to upload the 2 images that the person selects in the form, one to one folder and the other to another.

It then needs to add the record to the database including the image names.

The second page is a copy of this, the only slight difference is it is an edit/modify/update record page rather than an add page.
