Description:	We are in need of a web site form to be made. This web form needs to have a Submit button - that needs to have the form emailed in HTML or Text to 1 or a couple different email address. The form is made in basic HTML just to show what fields, drop boxes, radio boxes, etc. are necessary. This must be a QUICK turn around. If you are picked for the project you agree to: 1) Redesign the form that is supplied, into a professional, clean and user friendly look. 2) Make the form modern and have a fresh look. Clean, easy to read and modern fonts 3) Have the option on the form to email the form as HTML or Text. 4) Have the option on the form to enter the recipients email addresses. This area must be password protected, with the option to change the password and/or user name. 5) Keep the coding clean, and very well organized so we can at a later date go into and change questions, add questions, etc. 6) Must be Dreamweaver CS2 or CS3 friendly. The form is hosted on Godaddy servers. Happy bidding. Must provide FAST turn around for this project. Please see attached form.htm as to what content should be included.
