Description

I need a simple, yet secure web-form that can send attachments and uses CAPTCHA style image-security code. AJAX, or straight PHP are ok.

In addition to the 'browse' field for attachment, there will be only three fields to start (name, email, comments).

For the attachments it needs to have file restrictions (ie. "allowable extensions are: .pdf .doc" all other file types will cause an error. (for example only)

Results sent via email w/ attachment (no database)

This needs to be easy it set up (for non-programmer), have the ability to add additional fields if needed, and provide decent security against spoofing and flooding.

I need it ASAP. Thanks

I can have this done by tomorrow. I'll also use sessions etc. to prevent spamming and I'll make it extremely easy to add new fields. Allowing only certain file extensions is also not a problem. If you want more information, feel free to PM me. : )
