I will provide to you ALL HTML pages. Everything is designed and done for you. All you need to do is implement some programming (see below):
I need for you to add programming to a form that I have already created. You probably have to convert the HTML to PHP. It will work similarly to this example: http://www.nenmultimedia.com/weddings/bb/custom_package/contact.php?ss=1
However, it is MUCH SIMPLER and a lot less work for you. The only similarity is the form validation, continue/back function, submission of form to email and platform.
No database. Simple static php pages. With form validation, and upon completion, form is subitted to an email. And user gets an auto-reply.
There are 6 pages to the form. On the first page, the user can select which pages he will see. So, some pages in the form he will skip if he unselects the corresponding checkbox from the first page.
The first page will have checkboxes which can be disabled, cleared and grayed out if a radio button is not selected. And the last page has a dropdown menu that will hide or unhide depending on his selection. Plus it will either go directly to the "thank you" page, or will take him to "paypal" to process a transaction before the "thank you" page, all depending on his choice in the form.
