I am building an insurance lead form and I recently purchased and installed FORMfields lib v3 from formfields.com to help with my web form needs.
FORMfields in a framework for building web forms, here are the features http://www.formfields.com/features.php
I have already built a form that has all of the fields needed for my project but unfortunately with my limited knowledge of coding, I am having trouble customizing my form even though there are plenty of tutorials and help on how to accomplish these simple tasks.
