Description

What I am after is a checkout where everything displays upfront except for the form for 'ship to a different address' which will show only if you check the checkbox for 'ship to a different address'.

Also the login box to be on the right hand side, and it to accept guests by default.

I have attached a wireframe of what I am after. And another pic to show what I am after.

The maximum I will pay for this task is $40USD.
