Description

Hello providers, i am looking for LimeSurvey type survey system. I dont want anyone to copy the code or do anything illegal. I am looking for something similar but not exactly.

Please review the data model listed below that can be used:

If you have something similar / simpler, i am open to that.

Few requirements ...

- Must require log in for each survey
- Must provide a reporting interface to pull some meaningful reports
- Must able to generate 10 - 50 - 100 survey log ins that will be provided for clients
- Must have expiring user id / password component for clients who do not respond
- Must be able to run on php 4.5 (will give you the exact specs about this)
- Take a look at the admin app for the software ( http://demo.limesurvey.org/admin/admin.php )
- Take a look at the survey itself ( http://survey.limesurvey.org/index.php?sid=78184&lang=en )
- The questions / responses will not be super complicated .... and possibly pretty fixed so no need to worry about super duper complicated type of questions

Guys, we are looking for a simple approach to this. I will provide you with 10 questions. We can turn on or off those 10 questions and they can range from radio buttons, check boxes, free form text.

I am not looking for a system to sell to someone. Just looking for something nice and small which can be plugged into a site.

So, 10 questions (which i will give you) with responses coming in single (radio buttons), multiple (check boxes), or / and free form text.

Also, please keep your bids to a minimum.
