Description

I need a simple interactive form used to add/edit/delete questions and possible answers.
A client can add a question, select the answer type (radio, select box or text).
If the client selects either 'radio' or 'select box' from the drop down list, the option to add form variables appear. They will also have the option to add edit or delete options.

Client can add up to 5 Questions.

This project will heavy use javascript's createElement as well as css display properties.

Scripting instructions.
Post data will be saved as multi-dimensional array then serialized. Array will then be save in my sql db. On page load, if DB data exist, it will populate fields. Final project should consist of main file with PHP code to add/retrieve to/from DB.
