Description

To get an idea what i need take a look at attached picture...
I need no design, just the javascript that will make this work.

Besides this, there will also be inputs for:

email and phone (javascript validated)
note (needs no validation)

When user clicks SUBMIT, the data will be sent to server so that it can be emailed to me. Or he will be warned of minimum order amount.

Being able to lay the PHP part of my back too would be great...but if not, I will do it...

Example would be your best recommendation as this needs to be done today.

no database work needed, items will be hard coded into html (i will do that), everything will be on one page
that will load external .js

max budget $50

most items will have the same functionality, just different var name, so i guess one function with "this" could solve many things.
